Designing near and cohesive patient pathways together with patients, general practitioners, municipalities and regions

We strive to investigate and develop the Danish healthcare system together with patients, general practitioners, municipalities and the Danish Regions. On our tour of visiting community health houses in Denmark, we investigated the challenges of creating cohesive and near health offerings for Danish citizens. For instance, municipalities and Regions work under different legislation, have different IT systems, and are evaluated on different parameters. We have also had practitioners and leaders from municipalities and regions collaboratively generate inspiring ideas and concepts of a healthcare system where the patients feel safe and taken care of. And where patients are seen as whole human beings rather than only their disease.
Only by engaging these important actors as central experts in developing the future healthcare system do we get valuable ideas, insights and ownership to make our healthcare system one of the best and most sustainable in the World.

Pro-Active care for elderly people with dementia through sensor technology monitoring and detections (PACE)

The aim was to develop techniques to track changes in practices of elderly people with dementia (restlessness, falls, confusions, disorientation) based on synthesizing data gathered through sensor technologies, to prevent and reduce hospitalization by acting on changes on practice. Furthermore, the aim was to operationalize these data by co-designing in an easily accessible platform. A continuation of the project is expected in 2024.
The project was a collaboration between researchers from Department of Planning, AAU, researchers from DTU Compute, nursing staff from Skovhuset and Ryetbo and the company SIF.
